Would an envelope this size require you to fill out a customs form if it was going from the US to Canada? I usually just weigh things at work and put on the needed postage. No time to get to the post office lately...

Probably. Basically if it's bigger than a standard letter size and/or you don't want it to bend then yes it will need a custom form.
