This is the journal version of the original "5000 question survey" held by tenblackr0ses. Damilola from "My Journal Room" did this swap as an Internation and gave me permission to do a region USA swap of it. It's only USA due to mailing cost. Take/Make a notebook and write the questions inside and answer them one by one. You can add pics, cut out, doodle, colors,....whatever you like to beautify it, but generally it is about writing !
